We, the undersigned, have read and understood the rules and guidelines of the Multicultural Carnival. I/We agree to abide by all terms and conditions set forth by the organizers and commit to performing on the scheduled date and time. If kids are performing, parental consent has been obtained & agreed to the terms
Some groups may be entitled for a fuel reimbursement compensation which may be discussed with the organisers and agreed to prior to finalising the application.

Hereinafter referred to as Performer, and Eid Mela Carnival 2024 . In consideration for participation in the Event stage performance, the undersigned members of the group hereby agree to the following:
- Performer releases and discharges Eid Mela Carnival Team , its volunteers, agents, employees, directors and organisers, from all liability for all loss or damage and any claims of damaged therefore, on account of injury to any individual or property resulting from the Performer’s participation in Eid Mela Carnival.
- Performer agrees to indemnify and hold harmless Eid Mela Carnival Team , its volunteers, agents, employees, directors, and organisations, from any loss, liability, damage or cost, including attorney fees, arising out of Performer’s participation in the Eid Mela Carnival whether caused by the negligence or wilful act of the Performer or any other person or persons.
- Performer hereby assumes full responsibility for the risk of bodily injury or property damage relating to or arising out of Performer’s participation in the Eid Mela Carnival.
- The undersigned further expressly agrees that the foregoing RELEASE AND WAVER OF LIABILITY AND INDEMNITY AGREEMENT is intended to be as broad and inclusive as permitted by the laws of the State of Queensland.
6) The Organisers have the right to all photographs and video taken by our staff at the carnival.
